Medical Statement

If you answer "No" to all of the statements on page one, you need to only complete page one. Bring form to class with you on your first day of class.


If you answer "Yes" to any of the statements on page one, complete page one and then take the whole form to your doctor. You will need a doctor's release for scuba diving prior to entering the water. Bring the completed page one and page two of the form including Physician's Statement completed and signed by your doctor to the first day of class. The remaining pages are information for your doctor. If you are a doctor, you may not sign the Physician's Statement for yourself.

Open Water Certification:

• Release of Liability for Open Water Certification.

• Standard Safe Diving Practices and Statement of Understanding.
  
  

Advanced Open Water Certification:

• Release of Liability for Advanced Open Water Certification and Correlating Specialties.

• Standard Safe Diving Practices and Statement of Understanding.
